We provide you with information and answers to any questions you may have. Delaware Hospice is dedicated to responding quickly to meet your needs. We are here for you and your entire family. If hospice care is appropriate, and the patient agrees to Delaware Hospice care, the patient is admitted into our program with agreement from the physician.
Hospice care is specialized care that manages pain and symptoms. By working together earlier, we can concentrate on making the most of each day, focusing on reducing anxiety and providing comfort to the whole family.
A Delaware Hospice healthcare professional will meet with the patient and family as soon as possible or at their request. An experienced healthcare professional comes to the home, nursing home, assisted living facility, or hospital. At this meeting, we will discuss your greatest needs and concerns.
A nurse assesses the patient’s physical and emotional readiness for services, obtains a full medical history, and determines what medication and medical equipment is needed. All discussions are handled in a highly sensitive manner.
Once the assessment is performed, eligibility is determined by the Delaware Hospice Medical Director. A team is assigned to the patient and family, and a visit schedule and plan of care is established.
The nurse makes arrangements for medication and medical equipment to be delivered, at no charge, and continues to monitor patient care. The nurse also teaches the caregiver and family members how to care for the patient.
Members of the care team continually interact with the family to identify what is needed to offer the highest quality of care. The care team visits as needed.
