Qualifications/Duties:Through telephone triage, functions as a member of the interdisciplinary team to provide hospice services to patients and families. Utilizes the nursing process to provide quality nursing care to hospice patients in accordance with Delaware Hospice policy. Supports the primary care giver in the performance of physical care for the patient. This is an exempt position. Current license to practice professional nursing in the State of Delaware. Graduate of an approved school of nursing. Bachelor’s degree in nursing preferred. Certification in hospice nursing preferred. Minimum of two years of nursing, preferably in hospice or home care.
